Smart Health is a telemedicine project focused on the creation of a platform oriented towards at-home care for fragile patients, which allows at the same time for the management of healthcare information and the clinical records at the patient's home, the interfacing of an electromedical kit customised according to the specific needs of the patient, the enjoyment of a system of video-calling and online consultation of clinical documentation, assistance to patients in the implementation of care plans, all accessible through a system of access by reading an electronic healthcare card, which guarantees the management of information (and of said access) while respecting sector regulations.
The system, as well as allowing, with patient consent, access to visualisation of information by national healthcare professionals, also allows written access and modification of dedicated sections depending on the profile of the NHS user. The management of information occurs through peer to peer technology which allows access to the patient's machine on which all the information is allocated, only with patient authorisation. The project also provides for the development of an integrated wearable device, of reduced size, which allows monitoring of the patients through the collection of clinical information such as that collected by ECGs, pulse oximeters and thermometers. The information from the interfaced electromedicals is stored on the device and can be consulted, in the appropriate sections. Similarly, stored on the device at the patient's home, medical reports, instrumental exams and anything else necessary for patient care will be available. The system allows consultation of this information via the internet.
